First let us try to give a little overview about mesothelioma.

Mesothelioma is a form of cancer that is almost always caused by previous exposure to asbestos. This reduces sperm production, makes you less interested in sex and causes ED. The diagnosis of thyroid problems is not always straightforward. If you have any combination of these symptoms, it may indicate hypothyroidism:

• you feel tired most of the time no matter how much sleep you get;
• you begin to feel the cold more than usual;
• you find yourself increasingly constipated;
• you find that your thinking process are slowing down and you are growing a little forgetful;

The simplest test relies on the fact that the thyroid regulates the balance of calcium and phosphorous in your blood stream. We therefore draw a little blood to look at both the calcium and thyroid hormone levels. If the test confirms a thyroid problem, there are relatively inexpensive medications available to correct the balance. In more serious cases of hyperthyroidism in younger men, surgery may be considered.

If you are older, the treatment to balance your hormone levels should be done gradually. Older hearts may find the changes stressful. The beats may temporarily grow irregular and you may experience chest pains. And to dismiss one other possibility that may be lurking in your mind, the incidence of thyroid cancer is rare and, even if you are unlucky, these tumors are often easier to treat successfully than other kinds of cancer.

The study looked at 48 adult men, 34 with hyperthyroidism and 14 with hypothyroidism. So you're sitting there with all those questions still buzzing in your head about why you've suddenly found yourself in this situation with erectile dysfunction and whether a simple prescription for Viagra, Cialis or Levitra will solve the problem, where you can buy Viagra online, and so on.

The usual blood pressure test has come and gone, and now the doctor is gently telling you to drop your trousers. This is one of those moments you've been dreading and all those questions about Viagra are suddenly less important.

MS affects both the central and the peripheral nervous system. It therefore can produce a wide range of different symptoms. Treating them can make you feel more comfortable even though it will not stop the disease from developing. Some symptoms can be managed by changing your diet, and introducing more physical exercise and other changes of habit. Medications are available for each kind of symptom. Managing your condition involves deciding whether to treat any given symptom as needed or regularly, depending on how inconvenient or painful it is. The symptoms include:

• insomnia and fatigue;
• muscle spasms, stiffness and tremors;
• constipation and frequent urination;
• pain and abnormal sensations;
• depression; and
• erectile dysfunction.

There is an emerging consensus on how to treat MS. There is evidence that long-term treatment with interferon beta and glatiramer acetate can reduce or delay disability if started early enough. It seems that permanent damage to the nervous system is caused during the first phase of the disease. Early treatment can therefore improve the quality of life for those with relapsing-remitting MS. But some people whose initial symptoms are mild prefer not to begin interferon therapy, which causes flu-like side effects, until their condition worsens.

The treatment for the associated ED covers the full spectrum of remedies. The PDE-5 inhibitors, Viagra, Cialis and Levitra have good general effectiveness but, to date, only Viagra has been through clinical tests with MS sufferers. Nevertheless, since they all work in the same way, there is every reason to suppose that all three would help those with MS. But since none of these medications affects libido and depression with MS is quite common, counselling is almost certainly necessary to help couples resume intimacy. Since testosterone deficiency is not associated with MS, it is not helpful to prescribe a testosterone supplement. You should also note that the FDA has specifically warned against the use of Yohimbe for ED without prescription. Apomorphine hydrochloride sold under the brand name Uprima is used in the USA but, after a large scale study in the UK, its use there was discontinued in 2006.

Penile injections or suppositories that are inserted into the urethra are available. You can administer them yourself or your partner can do it for you. For those with MS, there can sometimes be a slight but transient ache from the injections. The resulting erection lasts for about one hour. There are three medications used for this purpose:

• prostaglandin E1 is sold under the names Alprostadil, Muse, Caverject and Edex - this has the best outcomes with the fewest side effects and is the only medication approved by the FDA for this purpose;
• Papaverine is an opium alkaloid used as a smooth muscle relaxant but it has a greater risk of causing priapism and so has not been approved by the FDA for this purpose;
• Phentolamine sold under the brand name Regitine is a vasodilator sometimes used in conjunction with either of the two other medications to improve blood flow.

As an alternative to drug therapy, you can also try the vacuum device which is placed over the penis. When air is pumped out, blood enters the penis and causes an erection. A small band is placed to prevent the blood from draining out and the erection is safe for about 20 minutes. If you have not responded to oral or injected medication, and have found counselling of little use, you may consider a surgically-implanted prosthesis whether semi-rigid or inflatable.

There can be associated problems with stiffness in the legs which make it difficult to maintain the necessary motion or to stay comfortable. Drugs, physiotherapy and experiment with different sexual positions may help. Similar management problems can occur if bladder control is not properly maintained. Again medications are available to keep the problem under control. Other problems of maintaining concentration and inhibition control can be addressed through counselling and behavioural therapy. Otherwise, different symptoms associated with depression and physical causes can be controlled but your physician must be involved at all times to ensure a proper and safe balance between the medications and your sexual needs.

Another from the other side of the fence.

Well, someone once said that the clothes maketh the man. Having put on my physician's hat, I found the change in point of view useful so I'm going to continue in that vein for another consultation.

We got you through the door as a straightforward patient suffering from erectile dysfunction (ED). In the light of the question and answer session and your medical history, we gave you the basic physical and, having found nothing to indicate more serious medical conditions, we discussed which of the three medications, Viagra, Cialis and Levitra, would work best for you. We also considered whether you should buy Levitra or any of the other medications online, looking at privacy vs confidence that you would get the medication you paid for.

But this leaves unanswered the question of what other tests might have been applied. We have all this laboratory space and skilled technicians standing by to do a range of tests. So, rather than leaving them standing around unemployed, let's run through a few of their tests.

We'll start with the sex hormones. We can test to see whether there are abnormalities in the levels of testosterone and/or prolactin in the blood. A low level of testosterone can make you less interested in sex and cause an erection problem. Research has shown that men with high blood pressure, diabetes or excessive body weight are twice as likely to have low testosterone levels. The body also produces less testosterone as it ages. This decline usually begins about the age of 40 years and slowly progresses over the next twenty years. It is sometimes referred to as the "male menopause" or "andropause", but the comparison with women is not justified. When women experience their menopause, ovulation ceases and oestrogen production falls rapidly over a relatively period of time.

For the majority of men, the testosterone levels decline naturally over time, but those levels remain within the "normal" range until death, causing no significant problems. In about 20% of men aged sixty and older, the testosterone levels fall below the normal range and produce testosterone deficiency (TD). In addition to growing older, TD can be caused by damage through injury or disease to the hypothalamus or pituitary gland. If the testicles are damaged, this can slow or stop hormone secretion and testosterone production. This latter condition is known as hypogonadism. TD can have a number of different effects:

• a general sense of tiredness and loss of energy;
• a loss of muscle mass and physical strength;
• a loss of libido or impotence;
• a decline in general awareness, memory and thinking processes; or
• anxiety or depression.

But it is important to recognise that other medical conditions such as liver disease, hypothyroidism and depression can cause these symptoms. Similarly, these symptoms may be the side effects of some painkillers, beta blockers and the medications used to treat anxiety and depression. Further, some men who are generally healthy experience one or more of these symptoms because of declining hormones other than testosterone.

For example a high level of prolactin (hyperprolactinemia) can be evidence of an adenoma (a small benign tumor on one of the glands in the brain). These small growths can affect other hormone levels in the body so that libido falls, ED emerges and infertility may result. When this problem occurs in women, a change in the menstrual cycle is a reliable signal of a problem, but many men are not aware of the problem until they have headaches or eye problems caused by the adenoma on the pituitary gland pressing against the optic nerves. So, if ED is reported, testing for prolactin can represent a test to eliminate a less common cause.

Returning to testosterone, there are a number of products on the market as "testosterone therapy". Sales are growing quite strongly. In 2004, pharmacies in the USA supplied 2.4m prescriptions for testosterone, doubling the number filled in 2000. No-one knows whether men or women are buying these medications, nor for what purpose. This is a controversial area because there is no scientific view on whether a slow decline in testosterone levels is an entirely natural effect of growing older or it represents a threat to health that must be treated.

Still wearing my physician's hat, it's time to reach a conclusion or two. There's clinical evidence to show that when younger men suffer any serious reduction in their level of testosterone, low sexual desire, fewer nocturnal erections and low ejaculate volume result. All of these problems are reduced with testosterone therapy. So, I am prepared to offer supplementation alongside Viagra, Cialis or Levitra in this kind of case.

But I prefer not to treat declining levels of testosterone in the average older man. There is no clinical evidence to link the age-dependent decline in testosterone production to any precise sexual symptoms, and when supplementation has been given in clinical trials, it has not produced any relief of sexual problems. Thus, if one of my patients has problems of ED and one of the factors is TD, I will prescribe Viagra, Cialis or Levitra, or recommend where to buy Viagra online as a temporary expedient while exploring other treatment options. But I see no reason to hurry to offer any other form of treatment for the TD unless the testosterone levels have fallen well below the norm for men of that age and body weight.

Rambler Media, a veteran of the domestic Internet, has changed its management. In an online interview, the new Financial Director, Artur Akop'yan, reveals where to invest money on the net.

The Internet holding Rambler Media is known to every Russian-speaking user of the World Wide Web. It's the first and the oldest Russian search engine. Search, news, an introductions service, mail on rambler.ru, the famous Lenta.ru, the goods catalogue on price.ru - for many years all these maintained Rambler as the market leader on the net. However, in the last few years, the company has been losing turnover. Certainly, the company has remained in the top three companies on the Russian net - Yandex, Rambler and Mail.ru - but reliance on old projects has become harder and harder to sustain, and fireworks from new ones - except the purchase of a block of shares in the contextual advertising company Begun - have not been remarkable. Their TV channel brought a series of losses, and was consequently sold. Mobile content initially generated decent profits, but then also began to stagnate. This spring, the company witnessed a change of all its top management. We invited one of the new team, Artur Akop'yan, Financial Director for the Rambler Media Group, to an online interview.

1. Can Rambler at last become competitive and marketable? Will it at last be possible to be proud of the company? Why does Rambler, unlike Yandex, not sell a full range of contextual advertising? How are you going to position Rambler?

- Rambler is a multi-service information Internet-portal offering a wide range of opportunities for work, information searching, communication and entertainment in the modern world. Rambler should be a portal that's convenient and interesting to use every day. As for marketability, the company's core business, its Internet division, has been profitable for several years already. In the future, we hope to bring you a number of pleasant surprises, so that you can be genuinely proud of us. We see our task as being to retain Rambler's market position. Our priority is to increase traffic and the number of users, and also to improve the quality of our services.

2. How successful do you consider the existence of the Rambler-TV television channel? Has the investment justified expectations?

- From a purely financial perspective, Rambler-TV was very successful. We sold it at a significant profit (in January 2007 a deal was finalized for the sale of the channel to Prof-Media Holding for $23 million, the initial investment was considerably less). The question of expectations in terms of audience ratings is not so relevant for us now. In 2006, Rambler's board of directors took the decision to focus exclusively on Internet development and, in that context, the sale of non-core business has been very successful.

3. Right now, hundreds of new companies are trying to "catch the spirit of Web2.0", to "create a social network", and are generally full of hope that a start-up in this field is going to garner then success. As a financial expert, and as a representative of one of the leading Internet companies in Russia, could you give your considered opinion on the prospects of such start-ups?

- As far as source of income goes, in a market like Russia's, the preference for business models connected with profit from advertising is going to be with us for a long time. For Rambler, and for other major players, it will be a long time before other means of attracting income (including direct provision of services), play more than an utterly insignificant role in our financial results. As a financier, when I hear of yet another plan to "create a social network", the main question that I want to ask is: how deep are the investors' pockets for this project?

4. I've got an idea for an Internet project, but surely if I take it to web-development specialists there's a good chance that they'll develop my idea without me, or work with me, but then create a clone, a perfected version, already knowing all the plusses and minuses of my project. How can I protect myself from outcomes like this? What is the minimal sum needed to create an Internet portal from nothing, and are there financial structures that might actually be interested in start-ups?

- As I see it, at the current stage of market evolution, to create an Internet portal from nothing, especially a portal for a large public with a horizontal line of services, is practically impossible, or at least demands the investment of tens of millions of dollars. On the hand, it's possible to develop a specialized service with the outlay of only a few tens of thousands of dollars. There are investors of that type right now, but, as far as I can tell, there are far more people interested in investing than there are realistic projects. As to your question about the protections of ideas: certainly there is that type of risk, and it all depends of the choice of partner. I should add, however, that it's very rare to find a project that is genuinely unique, and that offers something that can't be found anywhere else or can't be realized by other people.

5. It's often said today that there's more money in the Internet than there are interesting ideas, and that all more or less interesting projects are snapped up. If that's true, then what types of buyer are there around? What kinds of site generate buyer interest? Do the current profits of a project have decisive significance, or are the idea itself and the prospects for development more important?

- It's almost impossible to give a clear classification of the types of investor. Individual investors can be swayed by current profitability or by future prospects. To answer that question would require separate research. I'd like to warn against following general trends. There has been a lot of talk recently about social networks, about blogs, about instant-messaging systems and Internet messengers. In fact, the market for such resources is already saturated. The pioneers in that market have already had the chance to take the pickings, or to sell out to the biggest players, and further new investment in that type of project is pointless.

6. What do you think about SEO (search engine optimization)? How long will it take for search engines to come up with a personalized information search on the Internet, which will take into account individual requirements, search history, etc. And will that not spell the end for SEO?

- There have been several estimates given recently for the size of the market in search-result optimization. According to some of them, it has reached $50 million dollars per year in Russia and the CIS. It's a big industry. The battle between the human intellect and search engines reminds somewhat of the chess games with artificial intelligence (the battles between Kasparov or Kramnik and Deep Fritz or Deep Blue). Obviously, search engines are going to get more intelligent and more relevant through personalization (through narrower settings around users' interests), through more intelligent setting of parameters, through better quality filtration of spam (doorways), and through a host of other algorithms. But it's a little too optimistic to predict the end for search optimization. It'll get harder for them, but there will still be opportunities for flashes of human intellect.

7. What do you think of advertising on blogs, and how good are the prospects for this sector?

- It is possible to make money from your own blog, but not much. I would point to three basic means: contextual advertising, hidden marketing and sponsorship. For personal blogs in Russia, as a rule we're talking about tens or, in the best case, hundreds of dollars per year. As an example, take the recent noisy announcement of a contract between Soup Fabrik and Alfa-Bank ($50,000). Obviously, the commercial potential for that type of project can't be compared with the opportunities or budgets for media, banner or contextual advertising. There is only really potential for projects that have existed for more than a few years and have a strong original concept with specific content. Good examples are Alex Eksler's original project, where there are always numerous commercial advertisements. Also, Internet Things, the authors of which are looking for "favorite sponsors". Secret marketing is, of course, secret, so it's impossible to point to definite examples of its successful use. Although you could use the example of the recent post on the Norwegian Woodsman's blog about the Macdonald's factory. Most bloggers think of him as a hippy. There is an enormous public living in the blogosphere and, undoubtedly, that public is interesting to advertisers. But it's still questionable whether or not there exist the tools or even the spaces to meet the requirements of those advertisers.

8. How much longer are the prices for banner advertising going to continue rising?

- The Internet in Russia is only just beginning to develop, growth for Internet access penetration is forecast for the next five to six years, but it's already realistic to talk about the Internet having reached a critical mass to be of interest to advertisers. For now it is only 1.6% of general advertising budgets, while in developed countries that figure is closer to 5-6%. That suggests great potential for growth. Rambler is trying to unbalance the market by unnecessarily increasing prices for advertising. Nonetheless, thanks to the growth in our audience and the increase in interest in our advertising users, we have been able gradually and slowly to increase the price for a thousand displays.

9. What is the growth potential for regional and highly specialized segments on the Internet? Will they be able to escape from the shadow of big-budget projects like yours? As far as I can tell, they have two possible paths of expansion - to stop being targeted or to come under the umbrella of the strongest players on the Russian Internet.

- Highly specialized projects can live very well in their niche, if their concept is in demand. If the project is a copy of some competitor's and there is a strong desire to beat them, then the deciding factor is really the support of the portal. Regional projects are a different matter. In many respects, they exist in a different world. In fact, it's less that they need the portals, and more that the portals need them to increase their penetration in the provinces. From an economic point of view, access to regional and to national traffic in the regions is slowly leveling out, although nothing is going to change the fact that local resources know their area better, and that makes them more interesting to local users. The local listings business (a good American example would be Craigslist), undoubtedly has a definite potential. Neither Rambler, nor Microsoft, nor Yahoo! are capable of, or interested in, providing an information resource for the timetable of turning off the hot-water supply in Ussuriysk. And there are users who want not only to take pride in Olympic Games in Sochi in 2014, but also to find suburban train timetables for their local station, and cinema listings for their neighborhood. A resource like Rambler should consolidate those resources - through searches, through special projects, and through promotion. But we don't want to swallow up all regional resources and, what's more, we can't.

Make Money with your Blog

Someone asked on LinkedIn: "If I wanted to have a successful blog that potentially made money, what would I do?"

First thing's first, after figuring out what it is you're going to write about and designing the blog and hosting it somewhere (I would suggest purchasing a keyword-rich domain name rather than hosting it free on Blogger or some other blog site), the most valuable thing you can do is add buttons and links to your blog to allow users to subscribe to it in as many different ways as possible. Some people use services like Bloglines, some people use an RSS app, some people use Firefox live bookmarks, but not all of these people are savvy enough to know how to subscribe to a blog without a specific sub-to-this-blog link for their preferred RSS reader. A great resource to help you make these different links is Feedburner.com. Feedburner also allows you to design email signatures with animated headlines from your blog, and many other different ways to publicize and monetize your blog. Plus, it lets you watch the traffic and subscription trends for your blog's feed.

Next, you want to research keywords to find a keyword that relates to your subject matter that people actually search for, so that if you find yourself in the top ten rankings on Google for said keyword, you will actually get traffic from it. Once you find your keyword or key phrase, you want to make sure your blog's code is search engine friendly. You want to make sure your meta tags are within search engine guidelines, that your keyword density is appropriate for your subject matter, and that all of your titles have your keyword or key phrase in it. A good way to get a head start on this is to have your blog's backend hosted by Blogger, while the blog itself is published on your own domain. I say this because Google owns blogger and as such, the new version of blogger features extremely Google-friendly code.

After this, you need to get some links pointing to your site from good, Google recognized sites that relate to your subject matter. The best way to do this is to search on Google for your keywords or key phrase and ask the top ten ranking web sites to link to your blog. You may have to be creative about this, using what we like to call link bait. This can be anything that entices another web site owner to link to your site. Because your site will be new and most top ten ranking sites know a thing or two about search engine optimization, you probably won't be able to just get a link exchange - no one wants to link to a brand new blog for two reasons, one is that new bloggers have a tendency to lose interest after a while and you find yourself linking to a blog that never gets updated, and two is because the site has no Google PageRank. If you offer your potential linkers something else, you may have more success. For example, I once ran a campaign for a client that swapped links for a discount on my client's products. Whatever it is you swap for a link is up to you, be creative.

You want to add your web site to blog directories and blog resources such as Technorati. If you haven't spent literally a week straight doing this, you haven't found nearly all of them. There are so many! Your best bet is to search Google for a list of blog directories.

Add links to the bottom of every post that will allow readers to add to delicious, post to facebook, myspace, stumble upon, etc. Use as much 2.0 power as you possibly can. Get a Twitter account and tweet as much as you can, follow as many users as you can. Make sure every profile you've ever made for yourself or your business on any site, has a link to your blog. Make sure your blog address is everywhere, when you comment on others' blogs, when you talk in forums, etc.

You want to make sure you have an advertising policy available on your web site for potential advertisers to read, and as many different ways for them to contact you.

When all of this is set up and ready, it's time to send out a Press Release. Make sure it is enticing and boldly underlines how this blog is different and why people will want to read it. Submit your press release to as many press release distributors as possible. Again, just search on Google and you'll find a ton. Some cost money, some are free.

Now it's time to post. Make sure your posts have personality. No one wants to read a dry technical blog. You want to make sure your own personality comes out in each and every post. Also, the more controversy, the better. A controversial post will entice readers to send the link to their friends and family in either horror or pride - both are equally as good! It will also encourage comments and repeat visits when something heated has been posted to the comments and the poster wants to come back to check the responses. It will also encourage repeat commenters and some will be compelled to subscribe to your blog. Make it controversial enough and you may even hit the mainstream news, such as Michelle Malkin did recently when she objected to the scarf Rachael Ray was wearing in a Dunkin' Donuts ad.

Submit all of your article length posts to article databases utilizing the author's bio as a spot for a link back to your blog. Hopefully people will syndicate these on their own sites keeping the bio in tact and thus, creating more links to your blog.

Some people use traffic exchanges such as BlogExplosion or Blog Soldiers and while it does actually increase your traffic, most of that traffic is useless. The only one I have ever found that actually results in real readers, is BlogExplosion.com. These sites are not entirely useless, however, as they do have directories that your blog is submitted to when you join. And there are no obligations after joining, so I would suggest doing so just to get into the directories as they are even more links to your blog.

Make sure you blog regularly, but not too much. Many people subscribe to RSS feeds as a much faster way to get their daily reading in. Blogs with a lot of daily headlines, generally get skimmed and sometimes just deleted when there's no time. But blogs that only have a post a week, can tend to be forgotten about. My suggestion is to keep it to once daily, and at the very least, 3 or 4 times per week. If you are going to be unable to post for a period of time, vacation or something, post and let your readers know so that they don't think it's just another blog gone stale.

In the meantime, add Google Adsense to your blog, perhaps Amazon a-store and any high-quality affiliate programs that can relate to your blog's subject matter.

If you keep all of this up, you will have advertising revenue. The key though, is patience. Keep at it and don't get discouraged. Publicizing any web site can take months upon months before you see results. But once you do, it's highly worth it. You may even want to design and sell your own merchandise for further revenue

How to Write a Better Web Page Title

Titles are everywhere. Some people are defined by titles, some people loathe titles, and others like giving titles. Why do we do this? With so much information getting processed by our minds everyday, we need a way to categorize these groups of associated content in a quick referencing way.

This is the same methodology a search engine applies when it crawls a page. When you title a page, you are telling a search engine or user what the page is supposed to be about. We've all seen the one sentence summaries of television shows on the T.V. Guide. It allows for quick referencing and decision making by telling us what the show is going to be about.

This is part of the reason why naming your page titles is so critical to getting your site ranked. I can't tell you how many times I have seen a site that had one of the following two titles throughout the entire site: "untitled" or "company name".

While having the company name on all the pages could be a potential branding strategy, chances are, most visitors to your site don't know your brand name yet and are looking for the product or service that you're selling. You could incorporate the company name into the title, but it should be placed after the main message you want to convey about that particular page.

Every page should have a unique title. While you may have a subject that needs to be explained over a few pages, you should use the page title as an index of information for the reader. Sites that sell multiple products should have a unique title for every product. If you can't make the time to make each page stand out, why would you expect that page to ever appear in a search engine?

If possible, you should try to place your keywords in the title, since this will help the search engine determine where your page should be ranked. Remember, the search engines rank each page individually, that's why it's so important to use proper naming methods.

When you determine which keywords to use, go for a 4 to 6 word title when possible. After 58 characters, the title in the search engine results page will be truncated and will not be visible to the reader. Also, the longer the title, the less weight is given to each word.

The entire time you are conjuring up titles, always keep the reader in mind first. While some phrases may rank well in the search engines, a reader may never enter that phrase and you'll lose that targeted traffic you were trying to reach. Sometimes the phrases work out well for both the user and search engine, and in those cases success is usually quick to follow.
